Why are shin splint re-injury rates so high?

Shin splint re-injury rates are high primarily because the underlying causes of the condition are often persistent, multifactorial, and challenging to fully address. Some of the main reasons include; Incomplete recovery, poor running mechanics, training load errors (too much or too little), poor footear, running on poorly kept surfaces, lack of adherence to prevention strategies, pressure to return to sport, and cumulative stress. By addressing both the physical and behavioral factors, the risk of recurrence can be significantly reduced.

Why are shin splint injury rates increasing?

The increasing rates of shin splints (Medial Tibial Stress Syndrome, or MTSS) in athletes can be attributed to a combination of changes in training habits, environmental factors, and insufficient attention to prevention. Here are the primary reasons: Increased Training Loads

(Intensity and Volume). "No Pain, No Gain" Mentality: Athletes may ignore early warning signs of overuse injuries like shin splints, leading to higher injury rates. Growing Participation in Running and Endurance Sports, beginners in these sports often lack proper guidance, leading to training errors and poor biomechanics. Inadequate Recovery / Overtraining: Athletes are often training with insufficient rest, reducing the body's ability to repair microdamage in bones and tissues.

Busy Schedules: Balancing training with other life demands can lead to skipped recovery sessions, inadequate sleep, and poor nutrition, all of which increase injury risk. Surface and Footwear Issues Hard or Uneven Surfaces: Increased urbanization means more athletes are running on concrete and asphalt, which amplify impact forces on the tibia. Inappropriate Footwear: Wearing shoes without proper cushioning or support for an athlete’s gait can lead to overuse injuries. Insufficient Strength and Conditioning, weakness in key stabilizing muscles (like the calf muscles, tibialis anterior, or core) is common, especially in athletes who focus solely on their sport without cross-training. Increased Awareness and Reporting, awareness of MTSS has improved among athletes and healthcare providers, leading to more diagnoses. While this may not represent a true increase in cases, it contributes to the perception of higher injury rates. Younger Athletes in Intensive Sports, youth athletes are participating in high-intensity training at earlier ages, often specializing in one sport. This increases repetitive strain on developing bones, raising their susceptibility to shin splints.

What does the research say you should do to rehab your shin splints?

Rehabilitating shin splints (Medial Tibial Stress Syndrome, or MTSS) involves a combination of rest, strengthening, flexibility exercises, and gradual return to activity. Initially resting or stop activities causing pain, such as running or jumping, allows the inflammation and microdamage to heal. Typically, rest for 1-2 weeks, depending on severity. Although it doesn't have to be complete rest, during this time you can keep working on your fitness by cross-training. This would include low-impact activities like swimming, cycling, or elliptical training to maintain fitness without stressing the tibia. During this time you can also start strengthening key muscles of the lower limb such as; the calf muscles, tibialis anterior and intrinsic foot muscles. This is also a good opportunity to address and biomechanical issues you maybe having while running. However, one of the most important factors is to follow a structured plan that gradually exposes you to load. Lastly once your able to return back to sport it's important to keep up with some level of strength and conditioning to prevent reccurance.

What are my options to rehab shin splints?

Athletes have a variety of options to rehabilitate shin splints (Medial Tibial Stress Syndrome, or MTSS). A comprehensive approach combines rest, physical therapy, strength training, flexibility work, and adjustments to training habits. By combining these options and personalizing the approach based on severity and individual needs, athletes can effectively rehab shin splints and reduce the risk of recurrence.

can athletes get back to sport without doing physiotherapy?

While it is possible for athletes to return to running and sports without physiotherapy or strength training, this approach carries a higher risk of incomplete recovery and re-injury. Addressing the root causes of shin splints through strength, flexibility, and biomechanical correction is essential for sustainable performance and injury prevention. If avoiding formal interventions, athletes should at least incorporate basic self-care measures and gradual progression to improve their chances of success.

how can I tell if I have shin splints?

You can identify shin splints (Medial Tibial Stress Syndrome, or MTSS) based on specific symptoms, which typically develop gradually with overuse. However, distinguishing shin splints from other conditions like stress fractures or compartment syndrome is crucial. If the pain is severe, persistent, or localized, or if there are signs of swelling, bruising, or numbness, consult a healthcare provider. Imaging like X-rays or MRI may be required to rule out stress fractures or other conditions. Our training facility is open for coaching sessions Monday-Saturday with classes starting between 6:00 am and finishing at 7:30 pm.

How does this differ from standard physiotherapy?

The traditional approach for shin splint rehab revolves around session-to-session care; where you pay for a single consult, get advice, and return at agreed-upon intervals with your practitioner. Typically, the sessional approach does not support you with training and advice outside the consult, meaning you spend more time by yourself. Our approach turns that idea on its head — rather than paying each time you see us, you pay one fixed weekly cost, and you can see us as often as you'd like (some athletes are at our facility 5x per week!). This approach means that you get all the support you need, whenever you need it. This results in great shin splint rehab outcomes for our athletes and better support along the way.

How long does the shin splint rehab program go for?

In some cases, we do look after athletes remotely. If you live interstate or can’t make it to the facility, please reach out to us and we’ll assess if we can help. Most of the time, we’ll refer you to a coach we recommend in your city, because nothing beats coaching and accountability in the flesh. However, if you really want to work with us, we can explore that option with you, utilising telehealth, video coaching, and other remote technologies.

I don't like direct debit (the big commercial gyms ruined it for me). What are my options?

We totally understand. Ryan worked at big commercial gyms and understands how a lot of trust has been lost with gyms and direct debit (DD) memberships. If you don’t feel comfortable setting up a DD with us, prepaying for your shin splint rehab is still an option.
